11:00 AM-1:00 PM Registration – Church of the Blessed Sacrament

1:15 PM Candidates report to District Marshal– Saint John Bosco Room
1:30 PM Ladies Social – Haan Mansion Museum of Indiana Art
2:00 PM Exemplification – Holy Cross Room
4:00 PM Group and Individual Photos – Holy Cross Room
5:15 PM Formation of Dignitaries and new Sir Knights for Mass
5:30 PM Mass – Saint John the Baptist Catholic Church
7:00 PM Exemplification Banquet – Hardebeck Hall
FOURTH DEGREE MEMBERSHIP APPLICATIONS:
Membership Applications (Form 4) may be obtained from any Assembly, Council, Secretary to the Master, or District
Master. Applications must be COMPLETED IN FULL AND SIGNED by the following: Candidate, Proposer, the Council
Financial Secretary, Faithful Navigator, and Faithful Comptroller. Failure to obtain all five signatures will delay the
processing of the application.
Please ensure you have your signed 3rd Degree membership card with you on the day of the exemplification.
IMPORTANT: The original application Form 4 (all copies of Form 4) should be attached to the completed registration
form provided, along with a check payable to Richard V. Santangelo, Master, Indiana District and sent to:

EXEMPLIFICATION FEE:
The fee, set by the Supreme Council, for Fourth Degree Candidates (including Deacons) is $70.00. The fee includes a
social baldric, lapel pin, exemplification certificate, and one banquet ticket (whether you attend the banquet or not).
It does not include hotel accommodations. This fee does not include the yearly dues payable to your assembly.
Lunch (optional) for Candidates, Sir Knights, and Ladies will be at 11:00 PM. There is a $10 fee, payable on the
registration form.
ATTIRE
Candidates and Sir Knights must wear a Dark Suit (Black, Dark Grey or Navy Blue), plain white dress shirt – button
cuff (no French cuffs – no button-down collar shirts), conservative necktie, black socks, and plain black shoes. The
new uniform is to be worn as you would a business suit. Sir Knights may still wear the black tuxedo, black bow tie and
cummerbund, plain white pleated front shirt with a lay-down collar (no wing collars, frills, or color), black socks and
black shoes. Sir Knights must wear their social baldric.
Priests and Deacons may wear their clerical attire. Military Candidates may wear their full-dress uniform. Police,
Firefighters, and Emergency Medical Technician Candidates may wear their dress uniform.
The Color Corps must wear the new uniform with the respective regalia. File Leaders must wear the new uniform,
black tuxedo, or dark suit as indicated above.
Faithful Navigators, Past Faithful Navigators, Former Masters, Former Vice-Supreme Masters and Color Corps
Commanders may wear the jewel of their current, past or former office, or miniature Jewel(s) should be worn.
HONOR GUARD/COLOR CORPS:
Honor Guard and Color Corps will be under the direction of District Marshal Jeff Gadbois. Sir Knights possessing Color
Corps regalia may dress for the Mass procession. Formation for Mass will be at 5:15 PM.
LADIES SOCIAL & INFORMATION: The Ladies Social will take place at the Haan Mansion Museum of Indiana Art.
Transportation will be provided, and you will return with enough time to change for Mass.
Ladies attire suitable for Mass is appropriate for the day.
Please note that there is no hotel associated with this event, therefore all hotel reservations must be on your own.
